Insurance may be accepted for certain laboratory services; however, coverage is not guaranteed. Patients are responsible for verifying their individual insurance benefits and any out-of-pocket costs associated with laboratory testing.

Appointments may be canceled or rescheduled without penalty when at least 24 hours' notice is provided. Late cancellations and missed appointments may be subject to a cancellation fee or loss of deposit.
